Offering as much or as little assistance as required, our tailored care plans are created to fit each individual’s needs and aim to not only provide round-the-clock medical care, but wellbeing support allowing residents to live a rich and fulfilled life each and every day.

At The Hazelwell Care Home, we provide around-the-clock care in a luxurious yet comfortable setting. Combining the best of modern design and technology and supportive care and comfort allows staff to provide high-quality Residential Care in a home-from-home environment. The approach adopted by everyone at The Hazelwell Care Home is a personal one, tailored to each individual, focused on health, wellbeing and interests.

Staff continually customise and update each care plan to match the specific needs of each individual. Our teams of professional specialists work with residents and loved ones to create a bespoke plan that outlines every need and preference; all care plans are created to be adaptive, so should the needs or requirements change throughout their stay, our care continues to provide the highest level.

Personalised Residential Care

Every six weeks, care and support plans are reviewed at a Care and Support Meeting, at which residents and /or a representative will be invited to join to discuss your care requirements and the future steps surrounding them. We encourage the thoughts on any and every area of the care facility; our wonderful Home Manager continually maintains an ‘Open Door’ policy and is available for residents and their families at any time of the day.

Our purpose built care home offers exemplary standards of care and comfort to all our residents and day guests. With features including a chauffeur driven car, rooftop beauty salon, beautiful gardens and cinema room, each day brings exciting opportunities to be embraced by all residents.

Residential Care at The Hazelwell Care Home

From the moment you arrive at The Hazelwell Care Home our team is on hand to ensure that your stay is welcoming and comfortable. Prior to your arrival, our Home Manager will be delighted to have a chat with you to discuss your needs and requirements and tell you more about the wealth of facilities we have available to our residents.

You will be introduced to your key worker and a bespoke care plan will be agreed, our Chef will pop in to have a chat about what you like to eat and you’ll be guided round your new home to see all the facilities available.

We have a courteous, incredibly compassionate team who can assist you and customise your care plan as needed if requirements change over time.

When Is Residential Care The Best Solution?

If you or a loved one is finding it difficult to live alone, or if living alone is beginning to have a negative impact on your emotional or physical health, Residential Care may be the perfect option to consider. Residential Care, sometimes known as a “Group Living Arrangement”, can make a significant difference to the lives of our residents.

This type of care is ideal for those who occasionally require a little assistance in order to continue doing the things they like, but on the whole can manage living life largely unattended.

Families are always welcome to visit at any time and we always try to accommodate any special requests, where possible. Our Chefs offer a high standard of cuisine, catering to specific preferences and there is also an activity room available for residents, offering a balcony overlooking the beautiful Dee Estuary with views to North Wales.

What Our Residential Care Package Includes:

A wide range of facilities and supplemental services are included in the price of the stay at The Hazelwell Care Home. All residents have access to appropriate medical treatment, which is tailored to their needs. The following services are available

GP Visits

Residents will be able to schedule and attend GP appointments as frequently as required. Transport to and from can be arranged with the home. The cost of GP visits is included within the overall care home fee.

Frequent Dentist Visits

Residents have access to a professional dentist who looks after the overall oral health of residents and works with them to prevent and/or treat dental and oral disease(s). Check ups are included in your overall fee, with treatments available at an additional cost.

Optician Appointments

All residents have access to a professional optician where visual needs will be assessed, discussed and treated. This service is included within the overall care home fee (glasses however, come at an additional cost).

Chiropodist Access

We understand that some of our residents may require attention from a Chiropodist. For an additional cost, access to a Chiropodist can easily be arranged by one of our friendly team members.

Physiotherapy Sessions

Physiotherapy treatments, which can be regularly scheduled through a GP or privately, can help individuals regain their sense of self-identity and improve strength, flexibility and balance.
