The care continuum is the progression of care for a resident. This type of care focuses on opening lines of communication between all care providers who play a role in a resident’s care plan and ensuring that residents can continue to be cared for despite any changes to their requirements. 

Providing effective treatment across the continuum requires careful attention to detail as medical and wellbeing care may need to be adapted and therefore coordinated between a variety of providers. 

The best way to design these care “pathways” is by assembling multidisciplinary cross-departmental teams. When all of the care providers play a role in decision-making, the collaborative team can more effectively determine next steps for care delivery. 

Care providers include, but are not limited to:

  • The resident
  • The residents friends and/or family
  • Healthcare staff within the care home
  • Nurses within the care Home
  • Doctors within the care Home
  • Administrative staff within the care home
  • The kitchen team within the care home
  • External professionals such as GP’s, Physiotherapists, Chiropadists, Opticians and so on

Benefits of Continuum of Care in a Care Home

Evolving Care in One Setting

When residing in a dual-registered care home, residents benefit from being in a setting, surrounded by professionals who are able to care for a wide range of care needs. Should residents care requirements or progressive conditions change while living in a care home, the home is able to quickly respond, amend the care plan to reflect the current medical requirements. Continuum of Care at Hazelwell Care Home

We proudly deliver unrivalled care each and every day. The wellbeing of our residents is our top priority, and each resident is assigned a Primary Care Nurse, who will discuss and assess with them all medical and wellbeing needs. Follwoign on from this, a person-centred plan is created, and regularly reviewed. This Primary Care Nurse is responsible for ensuring a continuum of care is carried out effectively. 

All our staff are chosen for their maturity, warmth and caring attitude. We understand the importance of developing strong and long-lasting connections with residents and their loved ones is one that’s crucial to the way we work at Hazelwell Care Home. We place great emphasis on building relationships between our team members, residents, external professionals, and resident’s loved ones, helping us to learn a lot more about our residents and how to care for them most appropriately.

Hazelwell Care Home is a purpose-built, luxury home that is able to cater to a wide range of residents and support a wide range of medical needs. This year, we have proudly unveiled a brand new additional unit, ‘Gayton Nursing Community’. Named after our local neighbourhood of Gayton, we have created this community within our home to support residents with nursing needs, who have a variety of physical conditions. 

Here at our wonderful care home in Wirral, we provide a warm and safe environment for all of our residents who want to experience the advantages of group living and live as independently as possible, knowing their medical needs are in the safe hands of our wonderful professionals. We prioritise providing a varied range of first-class amenities and facilities that enable our residents to enjoy their lives to the fullest.
